Smiley face survey wording that works, and a smiley face survey template for the terminal or the email

The four-face scale is the most answered survey format there is, because pressing a face costs nothing and needs no reading. That strength is also the constraint: four faces carry about two bits of information, so everything depends on the question above them being about one specific thing. Get that wrong and you collect a large, confident record of people's general mood.

The question above the faces

Name the thing and the moment: how was your visit today, how was checkout, how was your appointment. A question about the business in general invites people to answer about the last thing that annoyed them anywhere, which is exactly the noise the format cannot filter. One thing, one moment, one press, and keep the wording identical for as long as you want the history to be comparable.

Four faces or five, and what the middle means

Four faces force a lean either way; five give a neutral. Neither is wrong, but a neutral option collects a lot of people who did not want to think, so if you use one, expect the middle to be large and read the ends. What matters far more than the count of faces is that you never change it, because a scale change resets everything you have collected.

The optional second tap

After a low press, one screen offering three or four reasons converts a bare complaint into something actionable, and it costs the customer a second. Keep the reasons short, concrete and mutually exclusive, and include an other option. This is the highest-value change most terminal deployments can make and it is usually free.

What a face cannot tell you

Who pressed it, why they pressed it, and whether they would like somebody to call. If any of those matter, the terminal needs pairing with an identifiable ask by email or QR code, which is the pattern most businesses end up with after a year of unattributed presses.

Questions people ask about smiley face survey

How many faces should the survey use?

Four or five, chosen once and never changed. Consistency matters more than the count.

What do we ask after a sad face?

One screen of three or four concrete reasons plus an other option. It takes a second and turns a press into something you can act on.

Can a smiley face survey work in email?

Yes, and it is one of the better email formats: the faces are clickable in the message itself, so the first click is the answer rather than a landing page.

Is there a smiley face survey template here?

The wording pattern is above, and the free worksheet turns your own footfall and press rate into the number of answers and follow-ups to expect.
